We offer Performance and Performance Plus as our group health benefit plans. Both provide local quality, choice and convenience for you and your employees. They also include referral-free access and flexible plan options. Pre-certification may be necessary for hospitalizations and some types of outpatient care.
With this lock-in plan, your employees get access to quality, cost-effective care in their community. They are only covered when they use the network and affiliated providers to receive care, except in emergencies.
With a Performance Plus plan, your employees get the same integrated network and benefit design as the Performance plan but with the added flexibility of more provider choice through out-of-network coverage. However, your employees will be responsible for more cost sharing through higher deductibles and coinsurance when they receive out-of-network care, except in the case of emergencies. Also, when out-of-network providers are used, your employees may be billed for amounts over the allowed charges (balance billing).
