Seton offers classes and individual education for diabetes education for Type 1 and Type 2 diabetes, gestational diabetes and pre-diabetes. Whether you are newly diagnosed or returning to learn new skills-medication administration, diet strategies or lifestyle strategies to improve your blood glucose control, or to address a special challenge in managing your diabetes, -our educators are here to support your path to better diabetes self-management.
Before your first appointment:
- Download and complete the questionnaire. Bring the completed questionnaire to the first appointment. Your educator will review this information before or at the beginning of the first class to tailor education needs. If you cannot print a copy, plan to arrive 30 minutes early to complete the questionnaire before your appointment. Bring a medication list and yesterday’s food intake (what, when and how much)
- Check out our locations and download a map if you need directions.
- Plan to arrive at least 15 minutes early to check in. Be sure to bring insurance card and a photo identification.
