Before your surgery, please read this guide carefully, follow all instructions and pre-surgery tips and provide all requested information. If you have questions after reviewing this guide, please call the Seton Northwest Hospital Surgery Department at 512-324-6163. If you call Monday through Friday between 6:30 a.m. – 4:30 p.m. and receive voicemail, leave your name and phone number and you can expect a call within two hours. If you call after hours, we will call you the next morning.
Please complete the Universal Medication Form (pdf) and the Patient History Intake Form (pdf) and bring them to your pre-admission surgery appointment. Provide your name as it appears on a government-issued identification card. The process will decrease registration time when you come to the hospital.
