Who Qualifies for the Program?

The program accepts patients with a wide variety of backgrounds and health problems. Anyone who resides in Travis County can contact STHP for help. The program targets the following patient populations:

• Patients with medical and/or social needs who require ongoing treatment but have difficulty accessing care or who do not see a doctor regularly
• Patients who have been admitted to the hospital or visited the emergency department at least twice in the last 12 months
• Patients diagnosed with congestive heart failure, skin ulcers, or sickle cell disease
• Patients who have sustained a serious injury requiring a stay in a hospital' intensive care unit (ICU)
