If you have been recently diagnosed with diabetes and you are not sure how to manage your care, we want to help!
To receive diabetes self-management training:
- A referral from your primary care physician is required if insurance coverage for education services is desired.
- Download the referral form.
- Have your primary care physician complete the form, sign it, and fax the form to 512/324-1396.
- Most insurance plans and managed care companies cover part/all the education costs. We are happy to discuss payment options for costs not covered by insurance.
- You may self-refer for education, but in this case, you would be responsible for applicable fees.
