Patient Load Forms
A Patient Load Form (PLF) is a tool which is utilized to notify the Office of Research Facilitation (ORF) of research participants within the Seton Healthcare Family Network. Submission of these forms ensures that items and services relating to clinical research are billed to the appropriate responsible party. In addition, PLFs also allow the ORF to report on research participation activity and demographics across the Network.
A member of the research study team must submit a PLF for each study participant who will be receiving clinical care or study-related procedures or services at Seton facilities within 48 hours of each study visit. This notification process is required regardless of whether or not billable services have occurred.
PLFs do not need to be submitted for chart reviews, database reviews, and/or data analyses studies.
Effective January 1, 2014, all studies submitted to the ORF will be reviewed to determine if PLF submission will be required on your study. If it is deemed by the ORF that your study requires the utilization of PLFs, a letter will be sent, via email, to the Principal Investigator and/or research team advising them of their obligations.

PLFs are submitted by logging into an Access database on the G Drive. Attendance to one of our weekly workshops is required in order to be granted access to the PLF database.
PLF database workshops occur every Friday in the Office of Research Facilitation located in the Clinical Education Center (CEC) at Brackenridge, Suite C3.400. You must sign up for a workshop prior to arriving.
