We will work with you to find out what you need and create a plan of action, but first we start with an assessment. Assessments are by appointment only.
What we offer:
- Compassionate and thorough level of care assessment
- Assessments provided by a licensed clinician
- Clinicians who will work with you to find the best resources to meet your needs
- Referrals if we do not provide what you are needing
Please note we do not offer medication management. We can provide referrals to psychiatrist if needed.

Where to go and what to bring:
- Please bring your photo ID, insurance card, and list of any medications.
- Please arrive 30 minutes prior to your assessment time.
- If you'd like, you can bring your completed patient information form to your appointment.
