The Seton Medical Center Williamson’s Release of Information Department is dedicated to processing your requests for patient-protected health information in a timely manner as well as protecting the confidentiality of our patient medical records. Hours of operation are Monday through Friday, 8:30 a.m. to 4:00 p.m.

To receive a copy of your medical record, an authorization to release medical information must be signed and dated by the patient or the patient's legal guardian. In addition, proper identification must be presented at time of the request.

Authorization forms can be obtained in the Health Information Management (HIM) Department located on the 1st Floor of Seton Medical Center Williamson or downloaded from this page (see right column).  For your convenience, you may mail the completed authorization form to us at the following address:

Please allow two weeks to receive a copy of your requested medical records. It is also important to note that some records are not complete until 30 days after discharge as they are awaiting transcribed reports from your physician.

Requestors may be charged a fee to cover the expense for providing copies of health information.  The fees are set by the Texas Health and Safety Code 241.154(e) and updated annually to reflect the most recent changes to the consumer price index as published by the Bureau of Labor Statistics.
