How to obtain your Health Information (medical records) at Seton Medical Center Williamson:

  1. To receive a copy of your health information, an authorization to release medical information must be signed and dated by the patient or the patient's legal guardian. In addition, proper identification must be presented at time of request.

  2. Authorization forms may be obtained and submitted in the Health Information Management Department, Release of Information section on the 1st Floor of Seton Medical Center Williamson. The direct phone number is (512) 324-4186.

  3. The turn around time to receive copies of health information is two weeks.

  4. Not all records are complete immediately after discharge. Some records are not complete until thirty days after discharge as they are awaiting dictation of history and physical, discharge summary, and operative reports.

  5. Requestors may be charged a fee to cover the expense in providing copies of health information. State and federal laws and regulations set these fees effective September 1, 2001.
