The Seton Asthma Center provides a disease management program designed to provide care coordination and education to people with Asthma and Chronic Obstructive Pulmonary Disease (COPD). The program provides asthma and COPD education to children, their families, and adults to help them better understand the disease process and to empower them to manage their Asthma and COPD on their own.

- Family and Individual Asthma, COPD and Smoking Cessation Education Class
- School Asthma Education Program
- Asthma Action Plans for care of asthma at school
- Coordination of asthma and COPD care between hospitals, doctors, schools and families
- Assistance with finding healthcare funding
- Assistance with finding a primary care doctor
- Home visits for asthma and COPD education
- Disease/Case Management
- Community Awareness Seminars in Neighborhood Centers
