- Seton will reimburse for one NCLEX review course.
- Online NCLEX review courses are eligible for reimbursement.
- The employee is eligible for reimbursement after passing the board exam.
- The cost of the NCLEX exam is not reimbursed by Seton.
- Please ensure that all sections of the reimbursement form are filled out and appropriate signatures are obtained prior to submission.

The following must be included with the completed reimbursement form:
- A copy of the review course receipt. Cancelled checks or bank/credit card statements are not acceptable proof of payment.
- A copy of the NCLEX board results (online results, copy of certificate, etc.).
- Fax the completed request form, a copy of the receipt, and a copy of the NCLEX results to 512-857-0403.
- Reimbursement is made within 1 – 2 payrolls; receipt depends on when in the pay cycle the completed reimbursement form and all documentation are received.
- Reimbursement is paid via regular direct deposit.
